Home Medicines Review

According to the Safety and Quality Council (2002), it is estimated that medication related problems result in 140,000 hospital admissions in Australia each year. It is believed that up to 69% of these are avoidable (Roughhead EE, 1999). The Home Medicines Review is an initiative that seeks to reduce these avoidable admissions through provision of a team approach to patient care.

The goal of the HMR is to maximize an individual patient’s benefit from their medication regiment, and prevent medication-related problems through a team approach, involving the patients GP and preferred community pharmacy, with the patient as the central focus. The reviews aim to achieve safe, effective, and appropriate use of medications by detecting and addressing medication-related problem/s that interfere with desired patient outcomes.

As of March 2006, a HMR facilitator has been employed through the KDGP to help establish the HMR initiative within the Kimberley. The program is currently under development. Details of progress will be updated on this website as they emerge.

The HMR Facilitator Scheme is a joint project of the Pharmacy Guild of Australia, ADGP and participating Divisions of General Practice and is funded by the Department of Health and Ageing.
